Wage: $28-$38/hr. 

Location: Canon City, CO.

Qualifications:

  • Must be an RN licensed in the state of Colorado or a Licensed Practical Nurse who is a graduate of an approved course in practical nursing and currently is licensed in the state of Colorado.
  • Must have a sincere ability and desire to work with the geriatric and rehab residents.
  • Wound care certification is preferred.

Principal Duties:

  • To administer treatments as ordered by the physician.
  • Take orders from physicians and execute them, including ordering medications from the pharmacy and documentation in the TAR and nurse’s notes as indicated.
  • Assist with diagnostic and nursing procedures if needed.
  • Prepare and update weekly nursing notes, wound progression sheets, weekly wound sheets and Braden scales in the EHR.
  • Assist with documenting regularly on specific residents designated at the nurse's station for primary and follow-up charting of but not limited to: incidents or accidents, refusal of care, status of wounds and changes of wounds, treatments and physician's change of treatment and notification.
  • Observe wounds and if signs of healing are not evident within a reasonable time, the physician must be notified.
  • Must keep the DON informed of skin assessments, increased risks or residents who are potentials for skin breakdown.
  • Make sure residents at risk have received preventative measures for skin breakdown such as but not limited to: low air loss mattresses, turned every two hours or as indicated, use of fracture bedpans when indicated, etc.
  • Observe areas with prosthetics for skin breakdown.
  • Prepare weekly skin report and review with the Director of Nursing.
  • Inform the Payroll Clerk of treatment supply needs on a weekly basis.
  • Assist in keeping supply closet clean and organized.
  • Complete an audit of the treatment cart monthly.
  • Complete any QAPI audits as assigned by the QAPI committee and/or Director of Nursing.
  • Report any audit concerns to the Director of Nursing and the QAPI committee.
  • Complete incident reports for skin tears and bruises.
  • Notify family and physician, and complete documentation in the medical record when on duty.
  • Consult with the wound specialist monthly, if needed, and change orders as recommended.
  • Complete the avoidable/unavoidable form, wound progression sheet and nursing notes on each decubitus.
  • Complete a thorough admission skin assessment on new residents admitted when on duty.
  • Trim fingernails and toenails of residents with a diagnosis of Diabetes and those receiving blood thinners when on duty.
  • Insert and change foley and suprapubic catheters as ordered by the physician when on duty.
  • Apply and monitor offloading devices as necessary and as instructed per care plan to maintain positioning, promote mobility and safety.
  • Answer call lights promptly and meet residents’ needs.
  • Keep chemicals locked up appropriately.
  • Complete other duties as assigned by supervisor.

Wage is dependent upon experience. We also offer $2/hr weekend differentials and evening, night and extra shift differentials if applicable.
