Imagine a job where your biggest win of the week is watching someone try something new, make a friend, or take a big step toward a goal they set for themselves. That's the heart of this role.

As our Supported Community Connections (SCC) Manager, you'll lead the programs that help the people we support get out, get involved, and thrive in the community. You'll be the energy behind the outings, the mentor for a team of Direct Support Professionals, and the trusted go-to for families and case managers. Your leadership will leave a lasting mark on the participants and on the team that supports them.

If you're a natural connector, a creative planner, and a coach who brings out the best in people, this is your next chapter.

What you'll do

Build a community full of possibilities

  • Discover and create community-based opportunities that match the interests and needs of the individuals we serve
  • Dream up fresh, stimulating activities that spark engagement and personal growth
  • Help people build natural supports and form safe, meaningful relationships
  • Cheer on participants as they work toward their goals and push past challenges

Be the connector for families and care teams

  • Serve as the main point of contact for families, case managers, consultants, and other authorized partners
  • Bring everyone together to shape Person-Centered Individual Service Plans (ISPs)
  • Make sure active treatment and ISP progress are documented and moving forward

Lead and grow a great team

  • Supervise, coach, and support Direct Support Professionals (DSPs)
  • Run staff meetings and monthly trainings, including orientations for relief staff
  • Keep schedules balanced so every program has the coverage it needs

Champion quality and independence

  • Use baseline data to shape programming and adjust goals as people grow
  • Protect every person's dignity, rights, and ability to make informed choices
  • Spot opportunities to make our services even better
  • Advocate for independence, choice, and the right of every individual to express their preferences

Keep it all running smoothly

  • Ensure programs meet Starpoint policies, Medicaid Waiver requirements, OSHA standards, and licensing and contractual requirements
  • Prepare and submit reports as needed
  • Join management and staff meetings to stay on top of best practices and organizational updates

What you bring

  • At least one year of experience working with individuals with intellectual and/or developmental disabilities, or completion of relevant training and certification programs
  • Strong communication skills: you can read and interpret safety rules and procedures, write clear reports, and talk comfortably with families, staff, and partners
  • Comfort with basic math, including percentages and ratios, and with making sense of simple data
  • Practical problem-solving skills and the ability to follow instructions in many formats
  • A valid driver's license with a clean driving record, plus a private vehicle that is licensed, registered, and insured
  • Current CPR/First Aid certification and any other training required for compliance

Why you'll love it here

We take care of the people who take care of others:

  • Paid Personal Time Off
  • Medical, dental, and vision insurance at minimal cost to you
  • Group term life and AD&D insurance
  • 100% employer-paid Employee Assistance Program for you and your immediate family members
