Job Description

Hildebrand Care Center is welcoming applications for a full time Social Services Assistant to join our person centered care team. We are an independently owned facility, and we love to have fun at work, while providing quality person directed care. The position is Monday-Friday. Must have at least one year of experience in long term care, preferably in a Social Services position. Prefer a certificate in Social Services. This position is a support to the Social Services Director, and requires an ability to work independently. Must be able to build positive relationships with our residents. Computer experience is required, and good interpersonal skills are a must. This position may require some transportation of residents out into the community. Pay is based on experience and education.

1.0 BASIC FUNCTION:

1.1 The Social Service Assistant assists the residents in meeting their physical, psychosocial intellectual and spiritual needs through social service programming and appropriate referrals to outside social service agencies, community resources and professional services. Assists residents, families and significant others with the transition into the facility.

1.2 This position is classified as Hourly, Non-Exempt

2.0 QUALIFICATIONS:

2.1 Must have a high school diploma or equivalent.

2.2 Long term care experience is required, preferably in social services.

2.3 Social services certificate preferred.

2.4 Must have a valid Colorado driver’s license with a satisfactory driving record.

3.0 PHYSICAL DEMANDS

3.1 Must be able to stand approximately hours, walk approximately 3 hours and sit approximately 5 hours during the course of the work day. Must be able to escort residents in wheelchairs. Must be able to occasionally bend, crouch, kneel, reach above shoulder/down to floor/across shoulders, stoop and twist. Must be able to frequently grasp with fingers and twist head.

3.2 Hearing must be adequate to be able to hear residents’ concerns, needs, desires, call lights, alarms, and calls for help and to answer telephone.

3.3 Vision must be adequate to observe residents and their condition, observe call lights, complete documentation on paper and computer, dial telephone, push wheelchairs, and ambulate from room to room. 3.4 This position is classified as low risk for blood borne pathogen exposure.

4.0 REPORTING RELATIONSHIP:

4.1 Reports to the Social Service Director.

5.0 AUTHORITY:

5.1 Has authority to assess departmental adherence to the organizational policies and procedures related to the Social Service department functions.

5.2 Has authority to communicate with other health related agencies to provide quality care and quality of life for residents.

5.3 Has the authority to access the electronic health record according to the HIPAA Privacy and Security policy as specified under clinical authorization.

6.0 RESPONSIBILITIES:

6.1 Responsible to assist the residents in meeting their physical, psychosocial intellectual and spiritual needs through social service programming and appropriate referrals to outside social service agencies, community resources and professional services.

6.2 Responsible to assist residents, families and significant others with the transition into the facility.

6.3 Responsible to use universal precautions and necessary personal protective equipment (i.e. goggles, gloves, mask) to protect residents, yourself and others from exposure to blood, body fluids and tissue.

6.4 Responsible to report any resident abuse to the authorities and the Director of Nursing as mandated under Senate Bill 13-11 and per the facility’s resident abuse policy.

6.5 Responsible to report incidents or exposure to the charge nurse immediately.

6.6 Responsible to follow all rules and regulations of the facility.

6.7 Responsible to maintain a neat and clean appearance.

6.8 Responsible to treat residents and staff with kindness and consideration, and to promote an atmosphere of cooperation and teamwork with the supervisors and employees of all other departments.

6.9 Responsible for the care and stewardship of all company assets and residents’ personal belongings.

6.10 Responsible for protecting the confidentiality of resident information and reporting all suspected violations of the (HIPAA) Privacy or Security Policy to the Privacy Officer or Security Officer.

6.11 Responsible for promoting excellence in person centered care by supporting the residents’ individuality through personal choices of daily life.

6.12 Responsible to help residents preserve as much independence as possible and to maintain their highest possible level of functioning.

6.13 Responsible to comply with attendance and timeliness standards.

6.14 Responsible to maintain a healthy, clean and safe environment, free from fall hazards, for the residents and staff.

6.15 Responsible to participate in emergency operations as need arises and as instructed by the Incident Commander.

7.0 ESSENTIAL JOB FUNCTIONS:

7.1 Welcome resident and family upon admission. Assist resident in calling family/legal representative if not accompanied or met at HCC by family/legal representative. Introduce to roommate if applicable. Set an appointment to complete admission social service forms.

7.2 Provide an oral and written explanation of “Your Right to Make Health Care Decisions” and obtain copies of Power of Attorneys, Guardianships, Conservatorships, Living Wills and other legal documents upon admission.

7.3 Assist residents and families in obtaining Advanced Directives as they request. Complete Advanced Directives forms with residents and/or legal representatives and review quarterly and as necessary.

7.4 Within 14 days of admission, complete BIMS and PHQ9 with resident. Complete social history, discharge form MDS and care plan with resident and/or legal representative.

7.5 Invite each resident, guardian and medical proxy to care plan meetings and other persons at the residents’ request.

7.6 Assess strengths, interests, quality of life desires and any concerns with residents and assist them and/or legal representative in obtaining quality of life items and resolution of concerns as able.

7.7 Visit residents who have been transferred to the hospital if possible and deliver or mail a completed transfer form and bed hold policy letter to the resident and/or legal representative.

7.8 Discuss eligibility requirements and provide contact information to apply for Medicare, Medicaid and VA benefits with residents, families and/or significant others; assist with redeterminations annually and as need arises.

7.9 Complete social service updates upon return from hospitalization, significant changes and annually.

7.10 Give residents insured with Medicaid, and/or their responsible party, written and oral explanations of writing PETI for new glasses, hearing aids, hearing aid repair, supplemental insurance, and dental work. Write PETI with residents/responsible parties, as they request and as needed.

7.11 Visit residents related to their psychosocial well-being and quality of life issues at residents’, families and staff request and as needs arise. Work on problem resolution as able.

7.12 Assist residents with calling family and friends at their request.

7.13 Work with resident, responsible parties and Activities staff in purchasing clothing as needs or requests arise.

7.14 Assist residents, legal representatives, laundry staff, C.N.A’s and Safety Committee in cleaning closets as needed.

7.15 Assume primary back-up status for the Social Service Director.

7.16 Answer call lights and meet residents’ needs within scope of practice. Seek assistance if unable to meet residents’ needs.

7.17 Model and promote person centered care, and actively participate in neighborhood meetings and activities.

8.0 NON-ESSENTIAL JOB FUNCTIONS:

8.1 Perform other duties as assigned by the Social Service Director.

9.0 MEASUREMENTS OF PERFORMANCE:

9.1 All assigned tasks are being handled on a consistently efficient and effective manner.

9.2 Resident, employee and peer feedback indicates that the Social Service Assistant is performing in a professional and consistent manner.
