Job Type: Full time, Hourly

Wage: $34-$39/hr.

Location: Canon City, CO.

Job Description:

Education, experience and licensing requirements:

-Must have an associate's or bachelor's degree in nursing from an accredited college or university.

-Must be licensed or registered in the State of Colorado. Must maintain an active Registered Nurse license in good standing throughout employment.

-Prefer 2 years of current experience in the MDS Coordinator position.

-Must have a current CPR card, and must keep current during employment.

Job Duties:

  • Monitor employees’ performance to meet nursing care standards.
  • Keep the DON informed concerning procedures and nursing care in the facility.
  • Contributes to the development of the philosophy, objectives, policies, procedures and job descriptions for the nursing service department.
  • Utilize the QAPI process/forms and data obtained from the Quality Indicators, etc., to identify, audit and implement systems to continually improve quality of life and care for the residents. Implement recommendations from the QAPI Committee as they relate to the Nursing Supervisor position.
  • Serve on committees as delegated by the DON i.e., Department Head Meeting, Resident Care Planning, PDPM, and QAPI meeting as requested.
  • Assigning schedule of all MDS’s for all departments in order to meet deadlines as required by CMS.
  • Prepare and implement Resident Care Plan Schedule in order to meet requirements of CMS. Direct the Resident Care Plan meeting in order to include all disciplines and provide a comprehensive plan of care for each resident.
  • Perform resident assessments for timely completion of MDS under the direction of a registered administrative nurse. Data entry of resident assessments into current computer program.
  • Perform routine audits of the MDS and make any necessary corrections. Collaborate with other departments regarding any errors that do not apply to nursing service.
  • Print and file all MDS and Care Plan documents in appropriate location. Purge old MDS and Care Plan documents as necessary.
  • Electronically submit MDS data and corrections to CMS on a weekly basis to ensure timely compliance with guidelines.
  • Review CMS (Matrix, Census and Conditions) reports and update as necessary.
  • Coordinate and implement resident care plans by reviewing charts and collaborating with nursing staff under the direction of a registered administrative nurse.
  • Review overall care plans and make recommendations as necessary to other departments.
  • Prepare monthly summary calendars monthly for nursing according to the care plan schedule.
  • Assist with fulfilling duties of the DON, Assistant Director of Nursing and Restorative Nurse in their absence.
  • Direct the PDPM meeting weekly in order to ensure compliance with Medicare rules and regulations.
  • Prepare and update resident bedside care plans.
  • Collaborate with QAPI Director to track and evaluate QI’s/QM’s in order to provide the best quality of care for each resident.
  • Contribute in the development and implementation of the competency fair.
  • Models and promotes person centered care, and actively participates in neighborhood meetings and activities.
  • Assume all the duties of a nurse, and work the floor as a med nurse or charge nurse as needed.
