Wage: $32-$40/hr. 

Location: Canon City, CO.

**3,000 sign on bonus**

ESSENTIAL JOB FUNCTIONS:

  • Perform daily rounds to observe and evaluate the residents’ physical, social and emotional needs, and to ensure quality of care.
  • Assist with the orientation of new employees, while promoting an atmosphere of welcoming and helpfulness.
  • Offer suggestions and assist the Human Resources Director in planning and presenting educational programs to meet the needs of the personnel and residents.
  • Make rounds with physicians and assist as necessary.
  • May take orders from the physician and execute them.
  • Order medications from the pharmacy.
  • Enter physician orders into the computer and record on the physician order sheet and progress notes as indicated.
  • Initiate baseline care plans on new admissions; make updates/changes to all residents’ acute care plans as necessary and report those revisions to the MDS Coordinator.
  • Assist the Infection Control Preventionist (ICP) with infection control applications and staff compliance, ensuring staff use of universal precautions. Initiate isolation procedures as necessary and report to the Infection Control Preventionist.
  • Report all exposure incidents to the Director of Nursing/ICP.
  • Utilize the 24-hour report to communicate concerns, census information and resident information including, but not limited to, addition of or change in safety devices, changes in residents’ conditions or needs, falls, infections, and addition of or change in psychoactive drugs.
  • Work in cooperation with the Assistant Director of Nursing to monitor and reduce the use of antipsychotic medication.
  • Work in cooperation with the Restorative Nurse and nursing staff on assessment, prevention, management and reduction of resident falls and alarms.
  • Administer medications, dressings and treatments as needed. Informs families of change of condition or behavior, medication changes and incidents.
  • Assist residents and families in coping with limitations imposed by illness and disability.
  • Listen to report of previous shifts and ensure that medication nurses give report to CNAs.
  • Works to replace nursing staff due to call offs as necessary.
  • Instructs and supervises CNAs in their duties and organization of their assignments, making sure resident care plan is being followed to ensure excellence in person centered care.
  • Supervises and mentors medication nurses in their duties and organization of their assignments, ensuring excellence in person centered care.
  • Admits, discharges and transfers residents following regulations and facility policies and procedures while honoring residents’ choices, dignity and comfort.
  • Night Shift Charge Nurse performs monthly med audits, initiates 24-hour reports, and completes supply lists daily.
  • Assess and ensure crash carts are properly equipped and in good working condition daily.
  • Assist with QAPI activities as designated by DON.
  • Lock and unlock doors as appropriate to maintain safety and security for residents and employees.
  • Keep lab sheets updated.
  • Draw lab work as necessary per physician’s orders.
  • Ensure chemicals are labeled and locked up appropriately.
  • Report medication errors to physician and DON immediately.
  • Utilize the SBAR, Know-it-All cards and the Stop and Watch tool to identify and report changes in condition to the DON and/or physician and document comprehensively in residents’ medical record.
  • Assess and comprehensively document on Medicare A residents every shift.
  • Attend in-service meeting, nurses’ meetings and other meeting as mandated.
  • Answer call lights promptly and meet residents’ needs. Perform other duties as assigned by the Director of Nurses or Assistant Director of Nursing.
