Position Summary:
The Resident Care Coordinator (RCC) oversees the delivery of high-quality care to residents in our assisted living community. This role combines supervisory responsibilities with hands-on caregiving and requires current QMAP certification. The ideal candidate is compassionate, organized, and skilled in medication administration, team coordination, and resident communication.
Responsibilities:
- Administer medications to residents as prescribed by healthcare professionals.
- Maintain accurate medical records and documentation for each resident.
- Monitor vital signs and report any changes in residents' health status to nursing staff.
- Assist with daily living activities, including personal hygiene, mobility, and meal preparation.
- Provide support and companionship to residents with memory care needs or developmental disabilities.
- Collaborate with nurses and other healthcare professionals to create individualized care plans.
- Ensure a safe and clean environment for all residents.
Resident Care Management:
- Coordinate and oversee individualized care plans based on resident assessments.
- Monitor changes in residents’ health and communicate with family members and healthcare providers.
- Maintain accurate documentation of care, services, and medical needs.
Medication Administration (QMAP Duties):
- Administer medications in accordance with QMAP regulations and facility policies.
- Maintain detailed and accurate medication administration records (MARs).
- Monitor for side effects and effectiveness of medications, and report concerns promptly.
Staff Leadership & Supervision:
- Train, schedule, and supervise care staff to ensure consistent delivery of care.
- Serve as a point of contact for caregiving staff regarding resident needs or concerns.
- Conduct regular team meetings and performance reviews.
Compliance & Safety:
- Ensure compliance with all state regulations, especially related to medication administration.
- Participate in audits, inspections, and quality improvement initiatives.
- Promote and enforce infection control and resident safety protocols.
Communication & Advocacy:
- Serve as a liaison between residents, families, staff, and medical professionals.
- Advocate for resident rights and well-being.
- Address and resolve complaints or concerns from residents or families.
Qualifications:
- Current QMAP certification in the state of [Your State].
- CPR and First Aid certification (or willingness to obtain).
- Experience in assisted living, memory care, or similar long-term care setting.
- Supervisory or team lead experience preferred.
- Excellent organizational and interpersonal skills.
- Proficient in documentation and care planning systems (e.g., EMRs).
