**3,000 sign on bonus**

Join an independently owned facility with a homelike atmosphere and an excellent team of caregivers, where we like to have fun while providing quality person-centered care! We are looking for an RN or LPN to work as a full time charge nurse. Shifts are twelve hour shifts. Night shift and weekend differentials. Main Duties:

  • Give meds and treatments as assigned and ordered by a physician, including all routes such as oral, rectal, gastrostomy tube, intravenously, topical etc.
  • May take orders from the physician and execute them.
  • Order medications from the pharmacy.
  • Enter physician orders into the computer and record on the physician order sheet and progress notes as indicated.
  • Utilize SBAR, Know-it-All cards and the Stop and Watch tool to identify and report changes in condition to the charge nurse and/or physician.
  • Report drug errors to charge nurse and physician immediately and complete a medication error report. Assist with maintaining facility equipment and complete work orders as needed for repairs.
  • Assist Charge Nurse with diagnostic and nursing procedures.
  • Assist with preparation and updating and following resident's care plan, MDS, monthly summary, treatment summary and medical record.
  • Maintain accurate and timely documentation as indicated for change of condition, behavior, incident or accident, refusal of medications or care, transferring or discharging of resident.
  • Assist with routine medication audits.
  • Give report to the C.N.A.’s at the beginning of the shift in order to promote quality and continuity of care.
  • Assist in monitoring and supervising duties and organization of nursing assistants in their assignments of, but not limited to, patient care, documentation, safety, dignity and privacy.
  • Monitor and chart behavior of residents on psychoactive drugs in the progress notes and complete psychoactive drug evaluations, and as needed on the monthly summary.
  • Night nurse to complete census sheet, 24-hour report and medication audits.
  • Attend in-service and staff meetings within the facility.
  • Assist the Charge Nurse as directed with the admission and discharge process.
  • Assist physicians as needed when in the facility.
  • Count narcotics at the beginning and end of each shift, and sign count sheets.
  • Report any abnormalities to the Director of Nursing immediately.
  • Listen to report of previous shift and give report to oncoming shift using pink acute care plans, and include information in report to C.N.A.’s.
  • Assist with the orientation of new employees.
  • Promote an atmosphere of welcoming and helpfulness.
  • Administer tube feedings as ordered by a physician.
  • Complete assignments and provide assistance as designated by Charge Nurse or Director of Nursing.
  • Keep med cart and nurse's station orderly and clean.
  • Monitor residents' room for cleanliness.
  • Be sure to check bedside commodes, wheelchairs, urinals etc.
  • Apply and monitor safety and/or therapeutic devices as necessary and as instructed per care plan to maintain positioning, promote mobility and safety.
  • Follow and monitor infection control procedures. Initiate isolation if condition warrants.
  • Perform basic nursing care as permitted in the nurse practice act, including but not limited to, foley catheter care and insertion, tracheostomy care, G-tube insertion, etc.
  • Answer call lights promptly and meet residents’ needs.
  • Keep chemicals locked up appropriately.

**Sign on bonus is for full time position only-$5,000 for RN and $2,500 for LPN**
