Job Description:

Chaffee County Public Health is a local agency that strives to promote the physical, mental, social and environmental health of the people of Chaffee County in partnership with community agencies. Under the direct supervision and guidance of the CCPH Director and Deputy Director, the Special Projects Coordinator will support existing partnerships by strengthening connections between community organizations and agencies, foster new relationships between CCPH and health stakeholders, coordinate activities to reduce fragmentation and address needs in the local health system, and integrate clinical and community-based strategies to address local health priorities, social determinants of health (SDoH), and health equity. The employee’s schedule should be flexible to support the CCPH Director and Deputy Director, to meet the increasing demands on local public health agencies, and to work on dynamic projects (funded and non-funded) that address local, and sometimes regional, health system gaps. Examples of programs include: Public Health Emergency Preparedness and Response (PHEP/EPR), Oral Health Program, Chaffee Community Clinic, The Happiness Project, etc. Some evenings and weekends are required. Position is grant funded and dependent on continued grant funds. This is a full time, hourly position with a wage range of $23.44 to $28.71, DOQ.
