Job Description:

Lake County Public Health Agency (LCPHA), an EOE employer, seeks a full-time provider (RN, PA, NP) in the Family Planning Program. The coordinator will perform a variety of public health services including assessments and education to ensure the health and welfare of Lake County families and communities. This person will supervise the Family Planning program, and will include clinics along with care coordination if needed, community outreach programs, IT responsibilities and grant responsibilities. This person will report to the Director. This position pays a competitive salary, depending upon experience. This is a full-time position that is eligible for benefits. This position is funded through the Title grant. If the funding is reduced or discontinued, this position may be reduced or discontinued.

Essential Functions

  • Advocates for clients and families while providing public health nursing to assure the availability and accessibility of health care services.
  • Collect, compile, and analyze data to identify needs for communities, families, and individuals as related to Family Planning.
  • Perform STI/HIV Screenings to target populations.
  • Performs pregnancy tests and provides council on pregnancy options.
  • Screens, councils, and dispenses various types of pregnancy prevention and birth control including emergency contraception.
  • Provides health teaching, education, referrals, and resources to promote Family Planning Awareness, prevent disease, and promote wellness for individuals, families, and community partners.
  • Perform communicable disease surveillance and education in coordination with state epidemiologists.
  • Works with the Public Health Director to collect, compile, and analyze data to identify needs and service gaps for communities, families, and individuals.
  • Provides consultation for the medical and health community.
  • Implements community health services and programs.
  • In collaboration with Public Health Clinical Staff provides clinic services including: immunizations and blood lead testing.
  • Works with Colorado Department of Public Health and Environment Family Planning Program staff to meet Family Planning Program requirements.
  • Builds relationships and collaborates with community partners including school districts and non-profits to create programming around sexual and reproductive health and violence prevention.
  • Participates in community health assessments and development of plans to address community health concerns.
  • Attends regular staff meetings, retreats conferences, committee meetings and state/local trainings for coordination and implementation of programs.
  • Participates in emergency preparedness and response activities as related to public health.

Requirements

  • Bachelor of Science in medical field such as Nursing. Certified Physician Assistant or Nurse Practitioner.
  • Associate Degree in Nursing with a bachelor’s degree in a related field will be considered. Associate Degree in Nursing with public health nursing experience or 5 years of general nursing experience will also be considered. Any combination of experience and training that would likely provide the required knowledge and abilities.
  • Must be a licensed Registered Nurse, Physician Assistant, or Nurse Practitioner in CO.
  • Valid Driver’s License.
  • Public contact experience in a service-related field with the ability to remain flexible and sensitive to a clientele diverse in age, ethnicity, and maturity.
  • Ability to work as part of a team.
  • Effectively communicate and work with co-workers, clients, other agencies, and the public.
  • Ability to assess situations and make prudent and appropriate decisions; ability to apply conflict resolution and problems solving skills to minimize negative personal impact.
  • Ability to efficiently plan, schedule, with strong organizational skills, with keen attention to detail and ability to complete quality work amidst multiple, competing priorities.
  • Superior verbal and written communication skills and effective listening skills.
  • Experienced in Microsoft Office (e.g., Word, Excel, and Outlook).
  • Current immunizations.
  • Completion of ICS 100, and 700, 800 courses within 6 months.
  • Current CPR for the health professional certification.
  • TB test, if not current, within 30 days of starting job.
  • Preferred proficient in English and Spanish.
  • Travel: May require some in-state travel for trainings.
  • Must possess and maintain a valid Colorado Driver’s License and the appropriate level of personal vehicle insurance.

All Lake County Employees may be required to work as a Disaster Service Worker when a local declaration of emergency or disaster is declared by the Board of County Commissioners. To be prepared for this role, all employees and emergency service function (ESF) leads will attend emergency management trainings and exercises as requested by the Director of Emergency Management. With advanced notification from OEM, employees will make reasonable efforts to attend training events in coordination with other employees and supervisors.
