Inpatient/Outpatient Float Pharmacist
Provide comprehensive pharmaceutical care to outpatient pharmacy patients, ensuring compliance with all applicable Colorado state and federal pharmacy laws and regulations. The pharmacist is a licensed professional responsible for the preparation, storage, and distribution of medications, as well as providing clinical services within the scope of licensure. Actively engage in medication therapy management, including drug utilization review and antimicrobial stewardship initiatives, offering evidence-based recommendations to licensed independent practitioners for inpatient care when applicable. Serve as the direct supervisor for pharmacy staff and oversee daily operations in the absence of the Outpatient Pharmacy Manager.
Key Responsibilities:
1. Maintain competence in both inpatient and outpatient pharmacy
duties.
2. Outpatient Pharmacy Duties
a. Performs initial and
finial review of prescription orders for accuracy and
appropriateness
including: dose, frequency, side effects,
interactions, and indications.
b. Review and fill prescriptions
accurately.
c. Be able to perform basic technician duties for
daily operations.
d. Use digital point of sale system.
e.
Ensure maintenance of efficient and current inventory.
f. Order
entry.
a. Appropriate screening for medication doses,
interactions, allergies, frequency and
awareness of potential
adverse drug interactions.
g. Maintaining records of inventory,
controlled substances, and any other record keeping
required by
the organization and laws.
h. Provide routine immunizations.
i.
Ability to adapt to changing workloads and flexibility when
interrupted.
j. Performs all daily pharmacist on duty
responsibilities in the absence of the Outpatient
Pharmacy
Manager. Including but not limited to:
b. Quality audits
c.
Preparedness for board inspections
d. Customer complaint
resolution
e. Staff leadership
k. Completion of pharmacy
related continuing education and training.
l. Employ tact and
courtesy at all times with customers, physicians, nurses, and
co-workers.
m. Maintain confidentiality of all patients and job
related activities.
3. Inpatient Pharmacy Duties
a. Performs
initial and finial review of prescription orders for accuracy and
appropriateness
including: dose, frequency, side effects,
interactions, and indications
b. Provides all acute care,
procedural, emergency, and standard medications for patients of
the
hospital
c. Provides IV admixture services utilizing aseptic
technique and ensuring medication
stability and compatibility.
d.
Be able to perform basic technician duties for daily operations.
e.
Performs appropriate medication order computer entry and patient
profile maintenance.
f. Screens appropriateness of all medication
doses.
g. Screens all medication doses versus patient renal
function.
h. Screens patient profiles for drug interactions.
i.
Screens patient drug allergies versus ordered medications.
j.
Ensures scheduling of medication to minimize patient adverse
reactions and maximize
efficacy.
k. Monitoring of patient
adverse drug reaction events.
Qualifications:
Education and/or Experience
Must have a current Colorado Pharmacist License (RPh) and experience in the outpatient/retail pharmacy setting.
Doctor of Pharmacy preferred
