Job Type: Full time, Hourly

Wage: $20.61-$$24.85/hr.

Location: Florence, CO.

Job Description

This position will be paid biweekly.

Job responsibilities include, but are not limited to:

Direct Nursing Care

  • Plans and coordinates basic nursing care and provides skilled level of nursing to residents.
  • Reviews and audits specific care activities to determine compliance with policies and standards of care as part of a department quality assurance activity.
  • Provides a broad range of medical and nursing treatments to residents as prescribed within scope of practice and determines most effective methods to ensure optimal outcomes.
  • Utilizes supplies and resources judiciously.
  • Notes changes in resident condition that need to be assessed by a registered nurse.
  • Documents treatments and gathers data to determine response to treatment.
  • Participates in work teams to establish best practice protocols for common treatments or procedures.
  • Administers specialized and routine nursing services such as medication and treatments.
  • Documents care provided in the medical record and other appropriate documents.
  • Evaluates residents upon admission and throughout the clinical stay when critical incidents, emergencies, change of condition or acute illness and behavior changes to identify nursing interventions.
  • Evaluates the effectiveness of the care /treatment given.
  • Explaining patient care before, during and after contacts; answering call lights; and assessing basic resident care needs; updating noted resident changes; recommending routines for patient and family.
  • Provide input and orientation for new employees.

Medication Administration and Assessments

  • Competently administers medications to residents within scope of practice while observing the five rights of safe medication administration, resident rights, infection control standards, and appropriate nursing policies.
  • Documents all medications given and justification for scheduled medications not given.
  • Appropriately identifies changes in resident’s conditions that need to be assessed by a registered nurse.

Infection Control

  • Provide infection control, implementing/using universal precautions, disposal of waste.
  • Clean linen room and equipment, stock supplies, complete supply forms.
  • Identifies high-risk residents for falls, skin breakdown, or other potentially harmful situations and assures appropriate interventions are initiated to correct problems.

Devices and Training

  • Identifies and corrects problems as appropriate.
  • Consults or communicates with other team members as needed to correct problems.
  • Maintain updated skills, knowledge, abilities and competencies necessary to the position through continuing education, on-going staff education programs, clinical supervision, literature review and participating in discipline and team development.

Minimum Qualifications, Substitutions, Conditions of Employment Appeal Rights

Minimum Qualifications:

High school diploma or General Education Diploma (GED) AND Two years of experience working in a hospital or healthcare setting.

Special Qualifications

  • Must maintain a Licensed Practical Nurse license in good standing.
  • While Colorado residency is a requirement for this position, licensure will be considered within Colorado or in any state participating in the Enhanced Nurse Licensure Compact (eNLC)

Substitutions:

Additional appropriate education will substitute for the required experience on a year-for-year basis.

* If using education as a substitute, you must attach your transcripts to your application. Employment history is calculated on a full-time basis (40/hrs per week). Part-time employment is calculated on a pro-rated basis to determine qualifications. Be sure your application specifically addresses your qualifications, experience, work products, and accomplishments as they relate to the position and minimum requirements.

Preferred Qualification:

  • Experience in a long-term care/assisted living environment
  • Experience working as an LPN
  • Relevant State of Colorado experience

Conditions of Employment:

  • Must be willing to submit to a background check
  • Tuberculosis (TB) test required.
  • Essential Services Designation - Must be available for call back to perform essential and/or emergency services of the agency without delay or interruption.
  • May be required to work overtime as a part of this position.
  • Shift work includes weekends, nights, holidays.
  • While every effort will be made for the schedule to reflect the shift/days agreed upon by supervisor and employee, employee may be scheduled on different days/shifts based on facility need.
  • Must possess or obtain a CPR card.
  • Former State employees who were disciplinarily terminated or resigned in lieu of termination must disclose the information on the application and provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position.
  • Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination will disqualify the applicant from future State employment with CDHS.
  • As a condition of employment, this position is required to be fully vaccinated for COVID-19. Continued employment is therefore contingent upon your receiving the first dose of a two-dose vaccine, or a single dose vaccine, within the first 30 calendar days of [first day of work] and becoming fully vaccinated based upon the timelines associated with the vaccination type. Until fully vaccinated, you will be required to follow all testing and PPE protocols. Be advised that “fully vaccinated” means two (2) weeks after a second dose in a two-dose series of the COVID-19 vaccine, such as the Pfizer or Moderna vaccine, or two (2) weeks after the single-dose vaccine, such as Johnson Johnson’s Janssen vaccine, as defined by the State of Colorado’s Public Health Order and guidance issued by the Colorado Department of Public Health Environment
