Sangre de Cristo Community Care is seeking a Full Time Licensed Practical Nurse (LPN) to join our team in Trinidad, Colorado. Responsibilities:
· Applies nursing principles and techniques in treating patients. Provides nursing care, emotional support, and education to the patient/family/caregiver.
· Maintains individualized patient care plans and records with accuracy and timeliness according to policy.

· Must have a valid non-restricted driver’s license.
· Flexibility and availability to work a weekend schedule if needed.
· At least 1 year of hospice or home health experience preferred.
