 Sangre de Cristo Community Care is seeking a full-time Registered Nurse to join our dynamic Home Health team in Trinidad, CO. Starting Pay: $35–$38/hour (based on experience)

Responsibilities:

· Plans, coordinates, implements, and evaluates the nursing care of all patients.

· Utilizes clinical knowledge and skills to identify patient needs and coordinate services and referrals to meet these needs

· Works closely with the inter disciplinary team to provide a continuous appropriate, comprehensive, and responsive plan of care for patients.

Requirements


  • A valid non-restricted RN license in the state of Colorado is required.
  • Must have a valid non-restricted driver’s license.
  • Flexibility and availability to take call for home health and hospice patients.
  • At least 1 year of home health or hospice experience preferred.
