Job Description

Starpoint has a long history of serving Fremont, Chaffee, and Custer Counties. Our Agency has provided case management services for over 30 years to individuals with a variety of needs. In May Health Care Policy and Financing Selected our agency to administer the Single Entry Point (SEP) Contract for Long Term Care Medicaid services in Fremont and Custer counties. In July 2021 Starpoint was selected to administer the Single Entry Point (SEP) Contract for Long Term Care Medicaid services in Alamosa and Saguache Counties.

JOB SUMMARY

This job supervises a team of Case Managers who are charged with promoting quality Health First Colorado member outcomes, optimizing member benefits, and promoting effective use of resources. The incumbent ensures care is medically appropriate, high quality, and cost-effective and is accountable for the oversight development, and maintenance of the department's case management processes. This includes utilization management, strategic planning, care cost initiatives, system development, compliance, and quality outcomes. Hires, trains, coaches, counsels, and evaluates the performance of direct reports. Supports an interdisciplinary approach to meeting members' needs through a strong collaborative relationship with external stakeholders.

PERSONAL SKILLS:

  • Ability to work independently.
  • Must demonstrate organizational and facilitation skills.
  • Must possess effective verbal and written communication skills.
  • Must be able to work with consumers, staff, parents, and community representatives in a team approach.
  • Must be able to work under stress and meet deadlines.
  • Must be able to work with minimal supervision.
  • Must be able to coordinate with families, supervisors, the general public, consumers, and agency staff in a pleasant manner.
  • Must exhibit mature judgment.

QUALIFICATIONS:

Bachelor's degree in social work, behavioral sciences, or related field with extensive experience in management as well as working with various community partners. Preferred M.A. or M.S. in human services or related field with five years of experience in case management. Must have a valid Colorado Driver's License and a certificate of personal auto insurance on file. Must have transportation and a phone
