Location: Canon City, CO.

This position is located in Canon City.

Position can be partially-remote/hybrid.

Strong preference to live within the 11th Judicial District (Chafee, Fremont, Custer and Park Counties)

SUPERVISION: Works under the direct supervision of the CYDC Coordinator.

RESPONSIBLITLIES: The Case Manager intensely supervises a caseload of juveniles who are eligible for CYDC services by monitoring a youth’s compliance with program conditions and orders of the court. The Case Manager is responsible for supervision of youth released from Pueblo Youth Services Center (PYC) and Marvin Foote Youth Services Center into a Colorado Youth Detention Continuum funded program and the community. The Case Manager is also responsible for monitoring tracking and electronic home monitoring (EHM) when applicable. The Case Manager serves as a resource for youth and families to assist in arranging and accessing appropriate services; communicates with courts and other professionals regarding the youth’s progress.

ESSENTIAL DUTIES AND RESPONSIBLITIES:

  • Provide Case Management services to all pre-adjudicated and sentenced youth released into the community on CYDC services.
  • Conduct intake interviews, complete behavior contracts with youth and parents to describe the program expectations and sign contract.
  • Conduct contacts with juvenile as determined appropriate by the juvenile court Judge and CYDC Coordinator.
  • Collaborate with other agencies involved with the youth including assessment teams, probation, attorneys, schools and counselors.
  • Refer client as needed to Multi-Disciplinary Team staffings.
  • Make service referrals to service providers for clients, as needed.
  • Prepare progress, incident or termination reports for court hearings.
  • In addition, prepare case update reports and provide sentencing recommendations.
  • Document all client contact in the Savio database within 24 hours of contact, including client compliance with program goals.
  • Communicate with outside agencies and maintain ongoing contact with service providers.
  • Stay informed about community resources available for youth and families.
  • Attend court on open clients when necessary.
  • Monitor EHM conditions/scheduling when ordered by the court.
  • Meet expectations of tracking program guidelines for frequency and types of contract.
  • Complete Juvenile Detention Screening and Assessment Guide (JDSAG) on all youth detained by law enforcement.
  • Conduct Colorado Juvenile Risk Assessment (CJRA) Pre-Screen of all clients within 48 hours of screening.
  • Enter screening information into the Savio Database and the TRAILS database at the time of screen.
  • Implementation of Youth Empowered Services (YES) Program for community service Facilitation of Life Skills Group using ARISE Life Skill Program for High School
  • Maintain appropriate paperwork, files and log books.
  • Conduct self in professional manner at all times.
  • Comply with all CYDC policies and procedure.

Essential Physical Functions

  • Must be able to lift, or raise up and down up to 25 pounds on a regular basis.
  • Must be able to communicate and exchange information verbally in a clear, concise and legible manner in English.
  • Ability to hear clearly to utilize phones.
  • Must be able to remain in a stationary position a minimum of 50%.
  • Manual dexterity for operating computer, calculator and/or other office equipment.
  • Mobility for inside office access to files, office equipment, etc.
  • Must be able to drive a vehicle for mobility around the judicial district.
  • Clarity of vision, with or without correction, for driving safely.

Requirements

  • Fully Vaccinated against the Covid-19 virus
  • GED or HS diploma
  • Relevant Experience preferred
