Wage: $18.34-$26.96/hr.

Job Description:

Starpoint is seeking a detail-oriented individual with excellent communication skills to coordinate a caseload of Host Home Contractors (HHC’s) and Respite Contractors. Our HHC's provide support to persons with developmental and/or intellectual disabilities within their home, and to assist them to live the most fulfilling life possible in a family like environment. 


Summary:

The Contracted Services Manager will create, implement, manage and oversee a quality program that meets the federally mandated settings rule as well as HCPF regulatory guidelines, and Starpoint’s host home policies and procedures while meeting the individual needs and choices of the persons receiving services.   As a Contract Services Manager, you will also oversee day-to-day operational functions of the Respite Program.  You will ensure all aspects of the program are operating efficiently and in accordance with Starpoint’s policies and procedures.  This position will also provide coordination and implementation of the Electronic Visit Verification system for Starpoint.


Responsibilities:

• Communicates with service coordination, HHC's, guardians and medical professionals.

• Conducts meet and greets with individuals, service coordination and guardians with potential new HHC’s.

• Prepares and understands the IPP process.

• Develops, implements, and maintains programs.

• Participates in on-boarding new HHC's including interviewing and selection.

• Conducts home visits to HHC's to ensure compliance with regulations and Starpoint expectations.

• Conducts environmental surveys.

• Documents activities and uses Therap


Requirements

QUALIFICATIONS:  

• Possess a B.A. or B.S in related Human Service field or have five years of related experience working in the Developmental Disabilities field.

• Must have a valid Colorado driver’s license and a certificate of personal auto insurance on file.

• Must clear back-ground check, DORA check and 3 valid reference checks

• Must be able to work scheduled hours and additional hours if requested by supervisor 

• Possess Qualified Medication Administration Person (QMAP) certification, CPR, First Aid, and Safety Care training. 

• Must have own telephone.  

• Own and utilize reliable transportation for all aspects of employment. 

• Willingness to travel to Colorado Springs, Pueblo, and throughout Fremont and Chaffee Counties.

• Demonstrate enthusiasm towards your job and team support.

• Demonstrate good reading and writing skills with good comprehension as well as demonstrate adequate math skills.
