Your Role:

What You’ll Do:
As a key member of our care coordination team, you’ll manage the entire referral process—from submission to closure. Whether it’s VA referrals or RFS (Request for Service) submissions, you’ll ensure everything is handled efficiently, accurately, and on time.

Your day-to-day will involve:

  • Coordinating with internal staff and external providers
  • Submitting and tracking all outgoing referrals
  • Ensuring timely follow-ups and resolving referral issues
  • Maintaining precise documentation for compliance and reporting

Qualifications:

EDUCATION and/or EXPERIENCE

  • High School Diploma required. Minimum three years related experience in a medical office, hospital setting, or clinic.
