Sangre de Cristo Community Care is looking for an experienced Certified Nursing Assistant to join our Hospice and Home Health team in Canon City, Colorado. Sangre de Cristo Community Care is the Southern Colorado leader in enhancing the quality of life and maintaining the integrity of individuals and their families in need of supportive or end-of-life services by offering symptom management, support, comfort, and compassion.
This position is Per Request Needed. Applicants must be able to work shifts per month and 1 Holiday a year. Starting wage is between $15-17/hr depending on experience
Responsibilities
- Provides patient care as directed by the RN or Case Manager.
- Participates actively as a member of the Interdisciplinary Team to assist in the development and implementation of the patient’s plan of care.
- Provides a full range of personal care through regular visits in patient’s home and in facilities in accordance with hospice and home health standards and the CNA scope of practice.
- Aid with personal hygiene, ambulation and exercise, and nutritional support for patient respecting personal and professional boundaries
- Responsible for providing patient care to Hospice and Home Health patients.

Requirements
- Must have a current and clear CNA license in the state of Colorado
- At least 1 year experience as a CNA.
- Home Health or Hospice experience preferred
- Must have a clear an valid Colorado Drivers License
- Ability to work a flexible schedule to include weekends and holidays
