Reports To: DAAS CMA Lead Case Manager and Supervisor (PRN)

ANNUAL SALARY: Position is a salaried position with an annual wage range up to $38,480.00 annually after successful completion of ninety (90) day probationary period. (Salary will be $3,142.53 per month during 90-day probationary period).

PRIMARY RESPONSIBILITIES

This position receives assignments from Home, Community, and Family Support Services (HCFSS) Lead Case Manager and/or Supervisor (PRN) for support services to assist Divisional staff in the provision of daily routine tasks and services. This position requires computer data entry skills so as to accurately enter required case medical information and CDASS monthly expenditure sheets into a state case management system. Position also requires management of case files; filing documents, completing forms, and making copies, as directed by leadership.

DUTIES INCLUDE THE FOLLOWING AND/OR OTHER DUTIES AS ASSIGNED:

Under supervision of the Lead Case Manager, this position provides clerical and administrative support to the unit supervisor and caseworkers, as well as technical support for the adult services programs as assigned. This will include assisting Unit Supervisor when requested with unit program needs. This may include assistance with unit reports and deliverables.

Position must be able to establish positive relationships with agency staff, community agencies and professionals, so as to gather needed information to complete a task.

Position will assist case management staff with provision of Long-Term Care Services. This will involve sending forms to physicians, the maintenance of client records, and completing required forms, as well as any other duties as assigned.

Position will require data entry and processing of state and local agency reports.

Position will be required to assist with organization and attendance for unit or department meetings and/or events; along with any new duties if unit rules or processes change.

Knowledge of the problems of the elderly, physically disabled, and other at-risk adults would be beneficial, as well as an understanding of services provided within the unit and agency.

Position may be assigned additional duties based on the needs.

ADDITIONAL REQUIREMENTS:

Must possess a valid driver’s license, reliable transportation, and specified car insurance.

Must be willing to travel to rural areas of the county and out of town as needed.

Must be able to successfully pass a criminal background check and drug screen.

Must be able to attend required out of town training and staff development activities.

Will be required to provide a driving history if a position is offered.

A professional and appropriate dress code is in effect for all employees of the office.

MINIMUM QUALIFICATIONS:

Language Ability:

Ability to read, analyze, and interpret complex documents. Ability to respond effectively to the most sensitive inquiries or complaints. Ability to make effective and persuasive correspondences via written and oral, to supervisors and leadership.

Math Ability:

Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.

Reasoning Ability:

Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.

Computer Skills:

To perform this job successfully, an individual should have knowledge of word processing software, spreadsheet software, payroll systems, human resource systems, database software, state defined case management systems.

Education/Experience:

High School Diploma or GED equivalent required. Post-secondary education in one of the human behavioral science fields such as human services, nursing, social work, psychology, sociology etc. is

preferred, but not required.

Six (6) months full time public contact and customer service experience preferred.

PREFERRED QUALIFICATIONS:

Associate Degree in Social Work or Human Service Related field from an accredited college or university or a degree in one of the behavioral sciences.

Five (5) years of direct experience in community outreach services.

Bilingual in English and Spanish helpful but not required.

Knowledge of geriatrics.

Medical knowledge and medical terminology helpful but not required.

PHYSICAL DEMANDS: Position requires substantial movement with respect to repetitious standing and sitting within the office area. On occasion, may be required to lift light to moderately heavy loads.
