Job Description

This position provides assessment and case management services to clients enrolled in the county’s self-sufficiency Colorado Works program. Determines employment barriers and the type and extent of such barriers. Evaluates the employment potential and available resources for achievement of self-sufficiency. Develops an Individual Responsibility Contract (IRC) with each client; provides or arranges for needed services; counsels clients regarding barriers to employment and monitors progress. Acts as broker in coordinating a full range of services designed to facilitate self-sufficiency. Maintains primary responsibility for monitoring progress of clients from intake to self-sufficiency. Develops with the client a written contract for achieving self-sufficiency based on specific short and long-term goals regarding employment, training, education, extended health care and child care benefits, child support and housing services. Monitors and modifies the contract and provides or arranges the supportive services needed. Interviews clients to determine barriers to employment. Makes referrals to appropriate resources such as medical, housing, mental health, employment, education, and training agencies. Provides counseling to clients regarding employment barriers, and makes referrals for family counseling, conflict resolution or other action necessary to provide for the best interests of the client and his/her family. Determine qualifications necessary for chosen occupation; compare client’s current qualifications with those needed to obtain chosen employment; develop plan of action with client to achieve necessary qualifications including attending appropriate education and training programs as necessary.

Qualifications:

Level I – Bachelor’s degree with a major in a human behavioral sciences or related field. Full-time, paid professional, high-level technical or para-professional work experience that provided the same kind, amounts, and level of knowledge obtained through the required education may substitute year for year for the Bachelor’s Degree

Level II - Minimum of one year full time, paid experience obtained in a social services agency at a Case Manager I Level

Additional Requirements:

  • A valid Colorado driver’s license, vehicle in safe operating condition, liability insurance.
  • Must be physically present within the geographical boundaries of Fremont County when performing on-call duties.
  • Must be able to pass a nationwide background check.
