Job Type: Full time, Hourly

Wage: $25/hr.

Location: Canon City, CO.

Job Description:

Position Summary: The Case Manager is responsible for managing a caseload of youth and their families who are dealing with the residual effects of complex trauma. This role involves assisting clients in transitioning successfully into family, school, and community settings.
Qualifications:
  • Bachelor's degree in social work or a related field, or equivalent work-related experience.
  • At least six months of work-related experience with at-risk youth.
  • At least 21 years of age.
  • Criminal clearances (Act 33 and 34, specific state and child clearances, and, if necessary, FBI).
  • Non-communicable diseases physical exam.
  • Valid driver's license from the employee’s state of residence.
  • Valid registered vehicle insurance.

Essential Functions:

  • Philosophy and Mission Support: Support the Abraxas philosophy and mission and promote the Seven Key Principles of care.
  • Crisis Management: Demonstrate appropriate use of Safe Crisis Management techniques and skills.
  • Client Supervision and Reporting: Supervise client activities and prepare required reports on client behavior and progress.
  • Court-Ordered Conditions: Support clients and their families in completing court-ordered conditions, including restitution, community service, and coordinating specific counseling needs and assessments.
  • Coordination with Agencies: Coordinate efforts with courts, probation departments, schools, employers, job training programs, recovery programs, counseling services, family members, and other social service agencies to meet clients' individualized needs.
  • Communication with Referral Agencies: Assist clinicians in maintaining regular contact with referral agency staff to ensure consistent communication and treatment for clients and their families, meeting contractual or licensing obligations and regulatory compliance.
  • Resource Development: Develop and maintain resources for clients, including community-based resource listings and curriculums. Facilitate daily group sessions for clients, providing a regular schedule of curriculums and informative materials.
  • Advocacy and Liaison: Attend and act as an advocate or liaison at client court hearings, school meetings, and reviews.
  • Transportation Coordination: Coordinate transportation for clients as needed for program or facility-related meetings and activities.
  • Scheduling and Visits: Develop a weekly work schedule to best serve each client. Coordinate with clinical staff to optimize client services. Arrange and conduct visits with clients and their families at the facility, home, and community-based locations before and after release.
