We are currently searching for a dynamic individual to become the CARE COORDINATOR for our unit. This position provides care coordination for the patients in the program.

All positions require pre-employment drug screening and background checks.

ESSENTIAL DUTIES AND RESPONSIBILITIES: include the following. Other duties may be assigned. 

Responsible for the development of care planning and patient needs assessments. 

Facilitates life skills groups by the CTP policies and procedures.

Acts as liaison establishing networks and collaboration with hospitals, community health centers, state, and human service agencies about patient services and needs. 

Document admissions, care management needs, discharge, and discharge planning. 

Administer case management assessments to identify patient needs and assist with assuring the Addiction Severity Index (ASI) is completed at intake. 

Organize a schedule of patient bed dates, transitioning levels of care, and consistent communication with the Medicaid Placement Coordinators. 

Assists treatment team with patient intake, transfer, and discharge assessments. 

Collaborating with the facility treatment team (medical, psychiatric, and SUD) in identifying patients’ needs.

Assist patients in meeting their basic needs (i.e. clothing, medical, judicial, religious, etc.) from available resources. 

Qualifications

QUALIFICATIONS

EDUCATION and EXPERIENCE:

An associate's degree or bachelor's degree is preferred (human services or related field).

The ability to obtain a Certify Addiction Technician (CAT) certification within 18 months.

Other:

Must have a good understanding of substance use, mental health, and treatment delivery systems.

Advanced computer skills are required.

CPR and first-aid certification. (The agency pays for initial training and renewals).

Must be able to take vital signs, recognize possible medical problems, and refer to the medical staff when necessary.

Valid driver's license and good driving record (must provide a current unofficial DMV driving record upon hire).

Able to communicate effectively in a multidimensional treatment program.

WORK ENVIRONMENT: The position is located in a residential setting.
