As a non-profit, Federally Qualified Health Center (FQHC), we provide premier primary and preventive care you can trust at our 34 service sites strategically located throughout multiple rural counties. 

 

We are looking for a Behavioral Health Associate to work at our Valley-Wide Canon City clinic in Canon City, CO. The Behavioral Health Associate will work as part of an integrative, primary healthcare network, using a Team-Based Care (TBC) approach to focus on holistic care for patients, across all ages with the goal to provide preventive, therapeutical and educational services. Fostering a culture of patient and family-centered care, service excellence and safety while enhancing patient, employee and community relations.

Job Responsibilities:

  • Provide clinical assessment, problem identification, need for medical necessity (DSM 5 criteria/justification), evidence based treatment modality, crisis intervention, individual/family/group therapy or other activities, case management, advocacy and outreach/prevention and interventions
  • Collaborates with the care team, following the Primary Care Behavioral Health (PCBH) model, developing and implementing evidence-based treatment plans and interventions through consultation, brief assessment, and psychoeducation.
  • Document patient interactions ensuring clinical charts within EMR system (NextGen) are kept up to date. Including patient intake, treatment plans, and other relevant information
  • Adheres to HIPAA and patient confidentiality at all times
  • If unlicensed, shall work toward completion of Colorado mental health license within 3 years of initially holding position.

Education and Qualifications:

  • Actively pursuing a Master’s or Doctoral degree in psychology, counseling, or clinical social work must have completed a minimum 1 year of schooling
  • Candidates may hold a current license with the Colorado State Board as any of the following: Licensed Clinical Psychologist (Ph.D., PsyD), Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Marriage and Family Therapist (LMFT)
  • Bilingual skills (English/Spanish) desirable but not required
  • Must be able to successfully complete drug testing and background screening
