Job Description

Are you passionate about seniors and helping Lake County seniors age in place? The Lake County Public Health Agency seeks an Aging Well Coordinator who will continue to drive forward opportunities identified in the 2017 Senior Master Plan. The coordinator will also ensure that all seniors in Lake County can age well in place while being: Surrounded by people they value Able to stay in a community where they belong Given opportunities to make meaningful contributions In optimal physical, mental, and cognitive health Free of injury or chronic conditions The Aging Well Coordinator will work with Senior Center staff to continue to provide transportation to medical appointments, social opportunities, excursions and be an advocate with community partners for seniors in Lake County. The ideal candidate will support programming that enriches Lake County senior's lives.

Essential Functions

  • Provide oversight of the Senior Center.
  • Oversee the transportation, social programming, and services for Lake County's aging population, including hiring and supervising staff.
  • Become familiar with the 2017 Senior Master Plan with the intent to seek opportunities that complement the findings.
  • Update policies and procedures as needed. Perform administrative duties for reporting requirements.
  • Responsible for all internal external communications, including newsletter, social media, Herald Democrat, etc. Develop and manage the Senior Center's budget and provide financial oversight of transactions.
  • Serve as the primary liaison to opportunities and activities that will be implemented by other agencies and community partners (such as the Parks, Recreation and Open Space Department, Lake County Build a Generation, Upper Arkansas Area Council of Governments, and Colorado Department of Transportation).
  • Manage and expand the Volunteer Program (transportation Help a Senior).
  • Seek out funding opportunities to support Aging Well Programs and Senior Center improvements.
  • Responsible for coordinating and managing rentals of the Senior Center.
  • Be a liaison to the Food Bank of the Rockies and adhere to regulatory requirements for food distribution.
  • Mange the Durable Medical Equipment at the Senior Center.
  • Serve on the Adult Protective Team for Human Services as needed.
  • Collaborate with other county, state and federal agencies to provide case management services to at-risk seniors.

Requirements

  • Bachelor’s degree or higher preferred in social work, psychology, public/community health, gerontology, or closely related field from an accredited university or college or any combination of education, training and experience which provides the required knowledge, skills, and abilities to perform the essential functions of the job.
  • May be able to substitute experience for education requirement.
  • Public contact experience in a service related field with the ability to remain flexible and sensitive to a clientele diverse in age, ethnicity and maturity.
  • Ability to work as part of a team and effectively communicate and work with co-workers, clients, other agencies, and the public.
  • Ability to assess situations and make prudent and appropriate decisions; ability to apply conflict resolution and problems solving skills to minimize negative personal impact.
  • Ability to efficiently plan, schedule, with strong organizational skills, with keen attention to detail and ability to complete quality work amidst multiple, competing priorities.
  • Must have strong written and verbal communication skills and effective listening skills
  • Must have a strong ability to analyze and understand data.
  • Must be able to use standard office equipment to include computer, scanner, fax machine, copier, and telephone.
  • Must have strong computer skills, including, but not limited to, an ability to use Microsoft Word and Excel, and to learn other programs as needed
  • Must be organized, flexible, able to work independently, and willing to do whatever is necessary to complete the scope of work.
  • May require some work at night and on the weekend.
  • Knowledge of the Lake County community, especially with regard to senior citizens, is preferred.
  • Proficiency in English and Spanish is preferred.

Travel:

  • May require very occasional in-state travel for trainings.
  • Must possess and maintain a valid Colorado Driver’s License and the appropriate level of personal vehicle insurance.

Work Conditions:

The majority of the duties are performed within an indoor environment. The incumbent may be exposed to some noise (electrical, mechanical, or human) on an occasional basis, and other related hazards associated with an office environment.
